​Does your primary care physician's referral have:

1. Reason for new referral (even if seen in past)
2. Imaging results (within 6 months at OTMH or on disc)
3. Treatment so far - physiotherapy, medication tried, other therapy
4. In-person musculoskeletal exam that matches radiological findings
5. Patient's contact information and health card #
6. Patient resides in regional referral LHIN area

• All referrals are triaged and your doctor’s office is contacted in up to 14 days
• Due to the OR booking times, your doctor's office will be notified that the referral has been received but that an appointment date is not yet issued until surgical times are put into Dr Ginty's schedule.
• Please ensure up to date X/rays and CPP with your referral
